This individual ensures a seamless connection between the WovenX platform and your procedural site’s internal workflows. They are responsible for receiving email notifications for procedure ready patients, confirming acceptance, downloading the patient’s Clinical Intake Summaries, and logging the confirmed procedure date in the WovenX platform. This should be someone involved in the daily operations of the practice.
The Clinical Intake Summary is a structured clinical document designed to support procedural clearance and scheduling for diagnostic colonoscopy following a positive CRC screening result. It is delivered securely through the Procedure Partner portal when a patient selects your location. Each Summary contains the patient's positive screening result and the clinical information required to complete clearance without an additional intake visit.
Review the Summary for procedural clearance and contact the patient to schedule their diagnostic colonoscopy. WovenX will provide expected response time guidelines separately.
To maintain program integrity and ensure each patient receives timely, quality care, we ask our Procedure Partners and their designated points of contact to adhere to the following:
- Contact Information: Maintain current contact information for your Primary and Secondary contacts in the portal, including direct phone lines, emails, and fax numbers. Contacts are expected to respond to WovenX inquiries within two business days.
- Review Requests: Render a decision (Accept or Decline the patient request to schedule a colonoscopy at your site) within two business days of receiving their Clinical Intake Summary.
- Patient Outreach: Initiate contact with the patient within three business days of acceptance.
- 6-Week Access: Offer at least one procedure opening within six weeks of the contact date. Patients may choose to schedule beyond the 6-week window, but a slot must be offered.
- Reporting: Record the procedure date in the WovenX platform within five business days of the scheduling. Update as needed if the schedule changes.
Upon receipt of the Clinical Intake Summary, the Procedure Partner’s site maintains sole responsibility for the final clinical review, obtaining any necessary procedural clearances from outside specialists, scheduling the procedure, providing all pre-procedural instructions and prep requirements, completing the procedure as well as any necessary follow-up care post-procedure.
The Fast Track intake is specifically designed to identify patients who are safe to proceed to procedure and do not require pre-procedure consultation. If a pre-procedure consultation is needed, a WovenX provider will attempt to complete the consult with the patient and will provide a procedure referral along with the consultation note as part of the Clinical Intake Summary. All clinical information will be provided to the Procedure Partner in the Clinical Intake Summary. The Procedure Partner still maintains final clinical authority to request an in-person consultation if required by the individual Patient’s circumstances.
Upon completing the Fast Track intake, eligible patients are presented with a list of local procedural sites based on their ZIP code. Once your site is live as a Procedure Partner, your facility will be visible and selectable to eligible patients within your geographic area.
Partners maintain full control over their procedural schedules. If your facility cannot accommodate a requested procedure, you may decline the scheduling request through the platform. WovenX will coordinate with the patient to find the next available partner site in their area.
To maintain participation in the network, procedural sites must be able to offer at least one procedure date within 6 weeks of contact with the patient, although the patient may elect to schedule out to a later date.
If a Procedure Partner has a pattern of declining patients, WovenX reserves the right to remove that Partner from the selection list until Partner is able to accept patients on a regular basis.
The agreement formalizes our shared commitment to HIPAA-compliant data security and establishes the operational standards necessary to close the colorectal cancer screening-to-procedure gap. The agreement clearly lays out the requirements for participating as a Procedure Partner and the situations that would trigger removal of a Partner from the network.
Yes. You may register up to 10 separate procedural sites (endo suites) under a single legal entity. In the onboarding form, you will be asked to provide addresses and designated Primary and Secondary Points of Contact for each location. The points of contact can be the same or different across multiple sites, but should be the correct accountable persons for scheduling and patient workflows for unique sites.
