There is no cost for practices or health systems to become a Procedure Partner. WovenX Health is dedicated to closing the colorectal cancer screening to procedure gap and provides the navigation infrastructure to connect you as a provider with local patients, at no cost.
WovenX Health delivers procedure-ready patients through a streamlined four-step process:
1. Patients with a positive test result complete the WovenX Fast Track intake to assess procedural safety regarding identification of potential anesthesia, cardiac clearance, and medication management needs.
2. Eligible patients select your practice from a list of partners closest to their ZIP code.
3. Procedure Partners receive an automated email with a secure link to the WovenX platform. There, you can access the Clinical Intake Summary, which includes demographics, insurance, and medical records needed for immediate chart creation.
4. Procedure Partner confirms acceptance and logs the procedure date within the WovenX platform, ensuring a seamless hand-off and coordinated patient support through the patient’s care journey.
WovenX Health provides Procedure Partners with access to a Clinical Intake Summary for each referred patient. The Summary includes a standardized medical and surgical history, clinical intake assessment, relevant procedure history, anticoagulation and medication status, and available patient records. Everything needed to assess procedural readiness and initiate scheduling.
Becoming a Procedure Partner requires only basic contact and procedural site information, plus a shared agreement that covers reporting, acknowledgement of scheduling requests, and a commitment to scheduling patients for colonoscopies promptly.
The frequency of referrals depends on the number of individuals in your geographic area completing stool-based DNA tests and subsequently selecting your facility from our list of local partners. You will receive an email with a secure access link to a patient’s Clinical Intake Summary the moment they choose your site.
Once a patient selects a Procedure Partner through the WovenX platform, the Procedure Partner performs the colonoscopy and has the ability to maintain an ongoing clinical relationship for follow-up care or future screenings.
No. WovenX Health is a specialty access platform that requires zero IT lift. Participating Procedure Partners receive patients through our secure web based platform, which does not require modifications to existing IT infrastructure or EMR workflows.
